ConvoForm
ActiveOpen source conversational forms you can host yourself
The right answer to exactly one question: can we run conversational forms without handing answers to somebody else. Nothing else in this directory says yes. The condition is everything a small open source project implies, a thin contributor base, no support commitment, and a roadmap that depends on volunteer time, so budget engineering attention rather than a subscription. If you have no self-hosting requirement, the funded products will cost less in total effort.
The response surface is a model-driven chat rather than a rendered field list, so the conversation is the product and not a skin over one.

Good for
- Keeping form answers inside your own infrastructure
- Teams with engineers who would rather own the stack than rent it
- Proving the conversational format costs nothing to try
Not good for
- Anyone who needs a support contract and an SLA
- Non-technical teams with nobody to run a deployment
