ConvoForm is the only Apache 2.0 conversational form builder in this directory whose code you can run on your own infrastructure with Docker, and its hosted version is free with no card. Conditional because its stack names OpenAI for the AI layer and Clerk for sign-in, and the repository does not document where answers go when self-hosted, so verify the data path before relying on residency. It is a small project with about 100 stars and no support commitment we could find: budget engineering time, not a subscription.
